As a social worker, nothing is more satisfying than helping a client meet all of his or her needs. Working in a subacute facility, one of the most important needs is developing an appropriate discharge plan for a resident; one that guarantees safety and comfort. Stephen from Golden Family GuidanceTM helped a recent resident of mine relocate to an ALF. I think I speak for her family when I say he went above and beyond to meet those needs. Not only was he compassionate and understanding, but he worked quickly. This resident's daughter came to me in crisis mode-her mother was previously independent and had taken a quick, sharp decline. Safety was a huge concern and she needed to find a place that would keep her mother safe without making her feel like a "nursing home prisoner." I happily passed along Stephen's business card, and within a few days the daughter called to tell me he had taken her to tour a few facilities and she had already chosen a favorite. The following week Stephen gave me all the necessary admissions paperwork, and within the next week she was moved into her new apartment. He coordinated all of the details, helped the family facilitate the move, and followed up to make sure it all went smoothly. Not only did I feel 100% comfortable referring this family to his services, but he was a pleasure to work with. He kept me updated throughout the entire process and it was seamless from beginning to end. I look forward to referring him to more families in similar situations in the future, knowing they will be in such capable, caring hands!
